Step 2: You will receive an email that you will have to confirm in your email account.
Step 3: After you have confirmed the email you will get a message that has the initial intake forms attached.
Step 4: Complete the in depth Initial Intake Forms…
- Your Initial Intake Form (Your complete health history – this is the most important document to fill out. The more I know the more comprehensive and adequately will I be able to create a personalized treatment program for you that will get you results.)
- Your Consent to Treatment Form (a legal document required before treatment) – you don’t have to sign it after I explained everything to you in the clinic.
- Your “How Healthy is your diet?” questionnaire (this will give me a good insight about nutritional status – Do you remember the saying “You are and become what you eat?”).
- Your Functional Pathology Testing form (This form is only needed in some cases) Just that you have it I attached it as a document as well).
- Your Metabolic Assessment Form this document will be downloaded via a link in the email that we will send after the completion of the form above. This form is an excellent process to check on the function of different organ systems on a symptomatic level.
Please bring all these forms completely filled out to your initial intake session. In this way you save time at the clinic and we can straight away dive into your case and treatment.
